California. health insurance question/concerns when I file my tax return. I had employer health insurance for 1 or 2 months in 2014 .?

I am in my late 20s. I live with my parents. I am a non-custodial parent. I LIVE IN CALIFORNIA. I was a substitute teacher for most of the year for 2014. I was a sub. teacher from Jan.-June; late Oct.-Dec. I was a regular (full-time) online teacher from early Aug. until late Oct. (my job was terminated in late Oct.) I received employer health insurance for 1 or 2 months. The rest of the months for 2014, I did not have health insurance (i.e. I did not have health insurance for 10 or 11 months). I worked for the (online teaching) employer from early Aug. to late Oct.. I know I received employer health insurance for one month from the employer (I believe I received employer health insurance for Nov., even though I was terminated in late Aug.). I could of received health insurance for Oct., but I am not sure. I know I did not receive health insurance for Aug. and Sep. I know I had the bronze plan, but I cant remember what insurance company insured me through my employer. I dont know if its bronze plan blue shield or bronze plan kasier or bronze plan blue cross,etc. I did get fringe benefits as well when I worked for the online school employer.I got dental and vision. On my W2 for the school online employer box 12a. has written in it DD and then it has written next to it (approx.) 1,400 Note: I did not receive unemployment. I do not think I would be able to prove I had health insurance for 1 or 2 months. I have no idea how to prove it. 1. what do I do before I file?

Hit a pole. Should I file an insurance claim or pay out of pocket?

We rent an apartment at a big complex and each unit gets a carport. If you can picture it, it's just one large, covered structure with two 4'x4' wood poles separating each port. Coming home, one of us turned into the carport but the turn was a little too short and the car struck one of the poles. Fortunately it did not damage the carport, but the estimate for supplies and labor for a new pole to be cemented and installed was between $300 and $400. They don't care if we pay it out of pocket or file with our insurer, as long as they get their money. I am finding this tricky. It's a little more than I want to pay out of pocket knowing I have liabillity insurance but at the same time I am concerned about the effects on our insurance rates if a claim is files. We both have several years safe driving records. Any thoughts?
